Application to Host Exhibition Match
Application to Host a Soccer Tournament/Event
Club Travel Policy
Please note, that if your team is going to deviate from the Club Travel Policy, you must have it approved by your Club Representative on record with the MSA (ie. President/VP). This is covered under the guiding principles of the policy.
Team Travel Application
Insurance: Affiliated members of the Manitoba Soccer Association (MSA) will receive a team travel permit (once approved by the League, Club and MSA) to cover injuries while participating in competitions outside of Manitoba. Essentially, a team travel application approved by the League, Club and MSA is the team's travel insurance for injuries sustained during competition.
Guest Player Release Form - Manitoba team
Guest Player Release Form - Non-Manitoba team
All players participating in tournaments or exhibition games on teams that are NOT from Manitoba must complete the above form.
PLEASE NOTE:
The MSA Team Travel Application and Guest Player Release Forms have slightly changed. The forms now require the Team to have approval from the League/Association first, their Club second, and the MSA last. This is to ensure the below elements are included in every application.
Every Team Travel Application should have the following documents:
-
League / Association approved team travel application
-
League / Association approved team roster
-
Club approved team travel application
-
Provincial or State Association documents that confirm the tournament / exhibition game(s) are sanctioned
-
League / Association approved > Club approved Guest Player Release Form(s) (if needed)
-
C.P.I.C. forms for all team officials (valid within 36 months of issue)
Regardless if you have already submitted a CPIC for another tournament/event, you still must include a copy with EVERY Team Travel Application you submit. Applications lacking the above elements will not be processed.
